Completing the Re-accreditation Workbook

After your initial 3 year accreditation is up and you wish to continue working as a sports trainer, you will need to fill in the SASMA Re-accreditation workbook
SASMA allows for a 6 month grace period after the expiry date of your Sports Trainer Certificate but failure to re-accredit during that period will require full attendance at a relevant course.
The Re-accreditation form can be found and downloaded on the SASMA Website: https://www.sasma.com.au/SportsTrainers/Reaccreditation.aspx
In order for your Re-accreditation workbook to be successful, you need to ensure you fill out the entire workbook correctly and send it in with the required documents.
Re-accreditation involves completing the following:
For Level 1 Sports Trainers:
- Completed Re-Accreditation Application Form including Payment Details
- Completed Education Record Form
- Copy of current First Aid & CPR Certificate (With at least 3 months until expiry)
- Signed Practical Competency Form (signed by any of the following: Level 1 Sports Trainer with a minimum 5 years’ experience as an Accredited trainer, Accredited Level 2 Sports Trainer, club Doctor or Physio, Head Trainer, Enrolled/ Registered Nurse).
- Copy of the Play By the Rules Certificate: Harassment and Discrimination and Child Protection
- Signed copy of the Sports Trainer’s Code of Conduct
- Payment Details
For Level 2 Sports Trainers:
- Completed Re-Accreditation Application Form including Payment Details
- Completed Education Record Form
- Copy of current First Aid & CPR Certificate (With at least 3 months until expiry)
- Signed Practical Competency Form (signed by any of the following: Accredited Level 2 Sports Trainer min 3 years’ experience, club Doctor or Physio, Head Trainer, Enrolled/ Registered Nurse).
- Copy of the Play By the Rules Certificate: Harassment and Discrimination and Child Protection
- Signed copy of the Sports Trainer’s Code of Conduct
- Copy of Assessment Questions
- Copy of Asthma First Aid Certificate
- Payment Details
